Medical History Form and Digital Signature

The Medical History form is used along with the secure eSignature feature from the Appointment Monitor.  Simply check in your patient and have the Medical History form appear along with the HIPAA privacy form for patients to update and acknowledge their information at a glance. 

MOGO's eSignature provides you with the security of a legally binding document, contrary to "just an image". 

The contents of the stored document are encrypted, along with true signature biometric data, such as measure pressure, speed and stroke.  Any document signed with the eSignature digital signature is secured saved for instant access without the risk of tampering.
